I think that the Indo-Pakistani Holocaust will see the use or destruction of all of Pakistan's and most of India's arsenal. It will end in heavy losses to India and the utter annihilation of Pakistan as a nation. The Indians will make sure of the latter by marching in ground troops and killing or driving out the survivors.
And Pakistan will fire the first shot. In full knowledge of all the information which makes me imagine this scenario. They will do so because they will be imagining that God will give them the victory. Yes, they're that crazy.
FWIW, I voted for "Another country", with the USA in mind. They have form for this, there having only been two nuclear weapons fired in anger, "Little Boy" and "Fat Man".
Of course, it's quite likely to be *launched* from the United Kingdom, but it'd be the USA that ultimately ordered the firing.
